Heart Worx is a Tamworth-based NDIS provider working alongside people navigating psychosocial disability, neurodivergence, and recovery. The work is human and unpredictable - which is why we don’t expect perfection. What matters most is being steady, present, and open to growth.

NDIS Worker Screening Check (or willing to apply)
First Aid certificate (or willing to get it)
A safe, insured vehicle for transport shifts

Lived experience of disability, trauma or mental health
Interest in gaming, tech, art, nature, music, or hands-on activities
Experience supporting people with social barriers
Weekend availability

Casual weekday rate from $43.31/hr (SCHADS Level 2)
Higher with qualifications/experience — up to $48.16/hr
Weekend, public holiday & evening penalties + super
99¢/km travel allowance
Paid fairly and transparently under the SCHADS Award

We’re currently holding space for one more skilled person to join our team in a dual Support Coordination / Psychosocial Recovery Coaching role.
This is a flexible employee position (full-time or part-time), based in Tamworth. You’ll be supported by a calm, kind team that values reflective practice, steady caseloads, and sustainable, person-led work.
SCHADS Level 4.2–5.1 (award-backed) based on experience
Approx. $90,000–$101,000 FTE + 12% super
Work laptop + phone provided
Flexible working days and hours (FT/PT considered)
Guidance and reflective check-ins for complex casework
A small, trauma-informed team culture with lived and professional leadership
Supporting participants to understand and coordinate the supports in their NDIS plan
Working alongside people with psychosocial disability and complex life experiences
Navigating systems like NDIS, housing, health, mental health, and community supports
Writing clear, trauma-informed notes and reports that reflect the person’s story and needs
Staying connected through regular check-ins and planning conversations
Collaborating with providers, services, and informal supports
You’ve worked in community services (Disability Employment, Disability Leadership, Youth Work, Case Work, Recovery) and can step into SC/PRC with confidence
You understand how the NDIS links with systems like housing, mental health, or justice
You’re confident with documentation and follow-up — or open to learning with guidance
You value calm, honest teamwork over hustle or pressure
You might bring lived experience, recovery insight, or professional coordination skills — we value all three
